Illness Scripts (Hallmark Features)

Definition
Illness scripts are mental models of diseases that clinicians build through clinical exposure & experience. It links the patient’s features into a coherent clinical diagnosis / differentials. Perhaps the most important tool in illness scripts are hallmark features – signs specific to that disease (e.g. absent ankle reflexes with ascending weakness in GBS, ejection systolic murmur radiating to the carotids in AS). Important tip – instead of learning every single feature of a disease – remember & apply 2-3 hallmark features that define the disease!
How to Apply
Initially, as medical students & beginner doctors – it is difficult to explain clinical features beyond textbook presentations. Pieces that do not fit the puzzle trigger a confusing list of extensive differentials with copious investigations (we’ve all started like this!).
